Please read this agreement before registering your child for a Green Oaks Athletic Club (GOAC) session. By checking the box at checkout, you confirm that you are the parent or legal guardian of the child being registered and that you have read and agree to the terms below.
1. Voluntary participation & assumption of risk
I understand that participation in athletic activities involves inherent risks, including the risk of physical injury. My child participates voluntarily. I accept these risks on behalf of my child and myself.
2. Release of liability
To the fullest extent allowed by law, I release and hold harmless Green Oaks Athletic Club, its coaches, and volunteers from any claim for injury, loss, or damage arising from my child's participation, except where caused by gross negligence or willful misconduct.
3. Medical authorization
In the event of injury or a medical emergency, I authorize GOAC staff to seek and consent to medical treatment for my child if I cannot be reached immediately. I am responsible for any resulting medical costs. I confirm I have disclosed any allergies or medical conditions relevant to my child's safe participation at registration.
4. Fees
- The session fee is $120 for the full six-week session, paid at registration.
- A spot is reserved only once payment is complete.
- The fee is non-refundable, including for any sessions your child is unable to attend.
5. Photo release
GOAC may occasionally photograph club activities for its website or promotional use. If you prefer your child not be photographed, email a coach before the first session and we will accommodate the request.
